Provides professional skilled nursing care directly and indirectly to patients primarily delivered on a visiting and intermittent basis in the patient's home. Supervises the care provided by other department staff members. OSHA Bloodborne Pathogens Class I. 

Provides skilled nursing care to patients in their homes. 
• Work in patient homes and occasionally in the office. 
• Stooping, bending and lifting to 100 pounds may be required. 
• Extensive travel including travel in inclement weather may be required. 
• Initiates plans of treatment.
• Provides services requiring professional nursing skills.
• Initiates preventative and rehabilitative nursing procedures for patients.
• Provides care appropriate for the neonate and the pediatric, adolescent, and geriatric patients served based on demonstrated knowledge of the principles of growth and development over the life span.
• Assesses and interprets data on neonate and the pediatric, adolescent,and geriatric patients' status to identify patients' requirements relative to their age-specific needs and provides care needed as stipulated in departmental policy.
• Records care delivered and prepares reports as required. 
• Prepares and maintains current progress notes on patients.
• Maintains records of home visits and submits charge information to the office.

• Informs, trains, and supervises other members of the patient care team. 
• Informs physicians and home health staff members of current and 
changing patient conditions.
• Provides education and training to patients, family members, and staff 
members.
• Supervises the work and patient care of LVNs and aides. 

Requirements
Education: Graduate of an accredited School of Professional Nursing. 

Personal Job-Related Skills: Effective verbal and written communication skills. Current knowledge of nursing principles, standards, and practices. Ability to independently manage multiple priorities. 

Licensure, Registry or Certification: Registered Nurse currently licensed by the state of Texas and a current driver's license. 

EXPERIENCE: 
1. Prior Work Experience: One year experience as a Registered Nurse and experience with Medicare and Medicaid preferred. 
2. Technical Training: Current in CPR. 

E. Physical and Mental Requirements: Duties of the position require moderate periods of sitting, standing and walking. The potential of some bending, stooping, reaching, pulling, and pushing when providing care to patients. Lifting up to 50 pounds and occasional heavy lifting in supporting or moving patients who may suddenly become weak or helpless.
