JOB SUMMARY:
Dalhart Senior Care Solutions is an Outpatient Behavioral Health Clinic for older adults and seniors, a subsidiary of Dallam-Hartley Counties Hospital District. The clinic is managed by Diamond Healthcare Corporation. Therapists are to conduct initial assessments determining whether a patient is fit for the program. Therapists are responsible for providing a therapeutic group and individual setting for clients with behavioral and mental health needs. Therapists are responsible for documenting patient progress as well as any relevant treatment notes
JOB QUALIFICATIONS:
Education: A Master's Degree in Psychology and have obtained a Texas License as a Professional counselor or Temporary LPC License. A Master's degree and have obtained a Temporary Licensed Master Social Worker Texas License or as a Licensed Master's Social Worker Level with intent to obtain supervision and become Licensed Clinical Social Worker Level.
B. Personal Job-Related Skills: The ability to effectively communicate orally and in writing.
C. Licensure, Registry or Certification: Texas Professional Counselor License. Licensed Master's Social Worker Level with current Texas License with intent to obtain supervision and become a Licensed Clinical Social Worker.
Experience:
Prior Work Experience: Previous experience in health care or behavioral health settings preferred.
Technical Training: None required.
Physical and Mental Requirements: Duties of the position require intermittent walking, sitting, and standing. Some bending, stooping, and reaching are required with the lifting of items up to a weight of 40 pounds. The ability to comprehend and follow routine written or verbal instructions or directions.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Complete required assessments with referred and current patients.
Biopsychosocial Integrated assessment I & II
Social History
Cognitive Assessments
Depression Scale
Discuss appropriate referrals with program director
Prepare patient for admission to the program based on the Diagnostic and Statistical Manual of Mental Disorders IV and V.
Determine possible diagnosis with psychiatrist's supervision.
Prepare and provide group therapy.
Groups will consist of psychotherapy with geriatric population patients requiring outpatient mental health support.
Document therapeutic notes
Take daily notes regarding daily group therapy and patient care following sessions and as needed.
Complete therapy
Individual and/or family as needed by the patient.
Monthly updated progress reports
Obtain signed patient release form.
Provide reports to primary care physicians and/or other agencies that are
Participate in meetings.
Treatment planning team meetings.
Communicate with telemedicine psychiatrist.
Report any information regarding patient treatment.
Complete master treatment plans.
Include short term and long term goals set for a therapeutic process with each patient individually.
Complete treatment plan reviews.
Review treatment plans during monthly team meetings with telemedicine psychiatrist.
Prepare and provide therapy.
Individual, marriage, and/or family therapy with behavioral heal patients.
Discharge planning.
Prepare referrals to needed community resources for continued care upon discharge from the Structured Outpatient Program, SOP.
Marketing
Market program to communities in our service
Offer training of mental health warning signs/needs for individuals
Train long term care facilities, community, civic, and religious groups.
Transportation
Assist with transporting patients when mental health tech needs assistance
OTHER SIGNIFICANT REQUIREMENTS:
Maintains confidentiality while working with confidential matters on a daily basis.
Interacts with patients, family members, medical staff members and other department managers on a continuous basis.
Provides service direct to patients while maintaining a positive customer relations atmosphere.
